Housing

Permanent Supportive Housing
Permanent Supportive Housing serves adults diagnosed with a serious mental illness who are experiencing chronic homelessness.
In order to be referred to Permanent Supportive Housing, please call the Coordinated Entry System at 540-853-1163.
Through the Coordinated Entry System, you will complete a common assessment so we can better understand your situation and level of need. Local service providers will then work together to match you with available housing programs. Because resources are limited, this system helps the process stay clear, fair, and efficient.
Projects for Assistance in Transition From Homelessness (PATH)
PATH connects participants with outreach services, screening and diagnostic treatment, habilitation and rehabilitation, community mental health services, substance use disorder (SUD) treatment, primary care referrals, job training, education services, and housing services.
