Permanent Supportive Housing

Permanent Supportive Housing serves adults diagnosed with a mental illness who are chronically homeless or have unstable housing. We also help frequent users of hospital or criminal justice system interventions.

Permanent Supportive Housing is usually part of a larger treatment plan. Availability can be limited, so it is best to ask your Case Manager what your options are.


Projects for Assistance in Transition From Homelessness (PATH)

PATH connects participants with outreach services, screening and diagnostic treatment, habilitation and rehabilitation, community mental health services, substance use disorder (SUD) treatment, primary care referrals, job training, education services, and housing services.

Who Is Eligible?

  • Adults with serious mental illness who are experiencing homelessness or who are at risk of becoming homeless (Ex: adults who are sleeping outside, in a community shelter, in a vehicle, or facing eviction)
  • PATH is not able to assist minors or adults with dependents in their custody/care.

For eligible clients, PATH may provide one-time financial assistance such as payment of one month’s rent to prevent eviction, assistance with first month’s rent or a security deposit to assist an individual with obtaining housing, or other related expenses.

PATH funding may provide clients with hygiene items, co-payments, outreach supplies, food, and clothing items.
