Requesting Medical Records?

• Before medical records can be disclosed the client, legal guardian, or Authorized Representative must sign an Authorization to Release Protected Health Information.
• If you are a current client of BRBH please contact your BRBH provider (case manager, clinician, therapist, etc.) to sign an Authorization to begin the process.
• If you are a former client of BRBH please call (540) 266-9209, leave a message and someone will return your call within 24 hours.
