long-term mental disabilities. The goal of a community support system is to
maximize each consumers potential for independence, improved quality of

Consultation and support to enhance linkages with
area assisted living facilities (ALF). Complete wrap-around services include
case management, ALF staff training, consumer education, medication
monitoring, physician/nursing consultation, skills training and supportive

Individualized community-based treatment,
rehabilitation, and supports designed to meet the needs of adults with
serious mental illness who often require 24 hour service availability to
avoid hospitalization. Complete wrap-around services include intensive case
management, psychiatric diagnosis and treatment, medication prescription,
administration and monitoring, Substance Abuse services, counseling,
vocational counseling, and life skills training.

Pre-vocation training, independent living skills,
socialization, and recreational activities provided in a member-operated
psychosocial rehabilitation program to persons who have psychiatric
disabilities. Education and support to families of adults with serious
mental illness is provided using the Mutual Education Support and Advocacy
(MESA) curriculum. Coordination and
consultation for church-sponsored Social Clubs, which provide opportunities
for fun and community integration to adults with serious mental illness
and/or mental retardation.
