Community Support Services programs for adults consist of case management, rehabilitation services, psychiatric services, community living services, and supports to consumers with long-term mental disabilities. The goal of a community support system is to maximize each consumer’s potential for independence, improved quality of life, and optimum level of functioning.

Case management activities include assessment, planning, linking, and monitoring services; crisis prevention and intervention services; and hospital and state facility admission and discharge planning activities.

Assisted Living Facilities Program

Consultation and support to enhance linkages with area assisted living facilities (ALF). Complete wrap-around services include case management, ALF staff training, consumer education, medication monitoring, physician/nursing consultation, skills training and supportive counseling.

Program for Assertive Community Treatment (PACT)

Individualized community-based treatment, rehabilitation, and supports designed to meet the needs of adults with serious mental illness who often require 24 hour service availability to avoid hospitalization. Complete wrap-around services include intensive case management, psychiatric diagnosis and treatment, medication prescription, administration and monitoring, Substance Abuse services, counseling, vocational counseling, and life skills training.

A combination of training and support activities designed to enable adults with significant functional limitations to achieve and maintain community stability and independence in the most appropriate, least restrictive environment.

Mountain House Clubhouse

Pre-vocation training, independent living skills, socialization, and recreational activities provided in a member-operated psychosocial rehabilitation program to persons who have psychiatric disabilities. Education and support to families of adults with serious mental illness is provided using the Mutual Education Support and Advocacy (MESA) curriculum.

Coordination and consultation for church-sponsored Social Clubs, which provide opportunities for fun and community integration to adults with serious mental illness and/or mental retardation.
